A member of the Isshi-20 (The 20 Doctors), an elite group of physicians from the Drum Kingdom (now the Sakura Kingdom). While he does not have a unique individual name revealed in the series, he and his nineteen colleagues functioned as a collective unit, recognized by their pink medical scrubs and surgical masks.

During the Drum Island arc, these doctors were central to the country's suffering, though not by their own choice. King Wapol had banished all other doctors from the island and forced the Isshi-20 to serve him exclusively, effectively holding the nation's health hostage to ensure the citizens' obedience. When Wapol returned to the island to fight the Straw Hats, the doctors were initially forced to follow him, but they ultimately showed their true colors by rushing to treat the injured Dalton and the citizens once Wapol's control was broken.

Following the defeat of Wapol and the liberation of the island, the doctors chose to remain in the newly named Sakura Kingdom. They now serve under the new King, Dalton, and work alongside Dr. Kureha. In the current timeline, they are free from tyranny and have dedicated themselves to democratizing healthcare, using their skills to treat all citizens rather than just the royalty.
